Job summary

Affinity Hospice Management LLC seeks a registered nurse to plan, organize and direct hospice care in home and community settings, drawing on community resources to meet the needs of patients and families.

Responsibilities include initial and ongoing comprehensive assessments, development and revision of care plans, medication administration, patient and family education, and coordination with the interdisciplinary team. On-call duties and maintaining a safe care environment are also required.

Qualifications

• Graduate of an accredited school of nursing.
• Registered Nurse license in good standing.
• Minimum 1 year of professional nursing experience within the last 3 years; BSN with 2 years’ experience in public health/home care/hospice preferred.
• Management experience not required; may supervise hospice aides.
• Contingent upon satisfactory references and criminal background checks.

Responsibilities

• Complete initial, comprehensive and ongoing assessments of patient and family to determine hospice needs.
• Provide professional nursing care using the nursing process.
• Develop and revise the care plan based on patient status and needs.
• Administer medications and treatments as prescribed.
• Counsel patient and family on nursing and related needs.
• Initiate and manage the plan of care with patient/family involvement.
• Participate in on-call duties and ensure equipment and services are available.
• Maintain a safe patient environment and coordinate care with the interdisciplinary team.

Education

Graduate of an accredited school of nursing

Registered Nurse license in good standing

Bachelor of Science in Nursing or higher (preferred)
