Affinity Health Management is seeking a hospice aide to work under the supervision of a registered nurse. The role involves providing personal care, promoting comfort, and documenting observations according to the patient’s plan of care.

Responsibilities include safe handling, mobility assistance, and supportive communication with patients and families. Care is delivered with a focus on quality, safety, and continuity of care in a hospice setting.

Competenze

• Completion of CNA course or equivalent training.
• Nursing Assistant Certified (NAC).
• Current Healthcare Provider CPR certification (hands-on).
• Current Food Handlers Permit.
• High school diploma or GED.

Mansioni

• Perform personal care tasks as assigned by the RN and follow the patient’s plan of care.
• Report observations and care performed to the RN Case Manager.
• Ensure safety and proper use of equipment.
• Assist with bathing, grooming, feeding, and mobility as needed.
• Maintain confidentiality and provide respectful communication with patients and families.
• Document care and changes in patient condition in EMR as required.
