Job summary

Affinity Health Management is seeking a dedicated Hospice Aide to support patients under RN supervision. You will perform personal care, monitor comfort, and document observations to ensure quality and safety in the patient's home or facility setting.

Responsibilities include assisting with daily living activities, mobility, and reporting changes promptly. A compassionate, reliable mindset and ability to work within a care team are essential for success in this role.

Qualifications

• Completion of a state-approved CNA program or equivalent.
• NAC certification and current CPR certification.
• Current Food Handler's permit and high school diploma or GED.
• Ability to read and follow written instructions and document care.

Responsibilities

• Provide personal care including bathing, grooming, and basic hygiene.
• Report changes in patient condition to the RN Case Manager.
• Assist with mobility, transfers, and range of motion as directed.
• Support with reminders for self-administered medications per state rules.
• Maintain a clean, safe living area and monitor skin integrity.
• Ensure confidentiality and compassionate communication with patients and families.
