Job summary

Affinity Hospice Management LLC is seeking a Hospice Aide who works under an RN to support patient comfort and care per the plan of care. You will report observations, assist with personal care, and help ensure safety and quality of care for patients and families in a compassionate environment.

Qualifications

• Completion of a state approved CNA course or equivalent training.
• Nursing Assistant Certified (NAC).
• Current Healthcare Provider CPR certification (hands-on preferred).
• Current Food Handlers Permit.
• High school diploma or GED.
• Ability to read and follow written instructions and document care.
• Understands hospice philosophy and comfortable providing specialized care.
• 1 year of hospice/long-term care experience preferred.
• Basic computer skills to document in EMR.
• At least 18 years of age.
• Licensed driver with insured vehicle in good order.

Responsibilities

• Provide personal care and assist with activities of daily living.
• Communicate with patient and family with confidentiality and respect.
• Ensure safety and proper use of equipment.
• Assist with bathing, grooming, feeding, and bed linen changes.
• Assist in dressing, undressing, and transfers.
• Support patient nutrition while adhering to dietary rules.
• Observe and report skin condition to prevent breakdown.
• Notify RN Case Manager of changes in condition or pain.
