Affinity Hospice Management LLC is seeking a hospice aide who will work under the supervision of a registered nurse to meet the patient’s personal care needs and promote comfort. The role includes reporting observations and providing compassionate, respectful care as part of a care team.

The aide will perform tasks such as personal care, safety support, mobility assistance, light housekeeping, and documentation in EMR, while adhering to state regulations and hospice guidelines.

Qualifications

• Completion of state-approved CNA course or equivalent.
• Licensed Nursing Assistant certification (NAC).
• Current Healthcare Provider CPR certification.
• Food Handlers Permit.
• High school diploma or GED.
• Ability to read and follow written instructions and document care.
• Understanding hospice philosophy and specialized care for the terminally ill.
• 1 year home health/hospice experience preferred.
• Basic computer skills to document in EMR.

Responsabilités

• Provide personal care and attend to patient requests promptly.
• Communicate with patients and families ethically and respectfully.
• Ensure safety and proper use of equipment (rails, O2, etc.).
• Assist with baths, oral hygiene, grooming, and changing linens.
• Assist in dressing, feeding, and mobility as assigned.
• Offer reminders for self-administered medications within state rules.
• Help with bedpans/urinals and light housekeeping tasks.
• Monitor skin condition and report concerns to RN Case Manager.
• Support in transfers/range of motion as directed by nurse or therapist.
• Document care and participate in quality improvement activities.
• Maintain patient living area in clean, orderly condition.
