Job summary

Affinity Health Management is seeking a Hospice Aide to support patients under the supervision of a registered nurse. You will perform personal care, assist with daily activities, monitor patient comfort, and document observations in the plan of care to promote quality, safety, and continuity of hospice services.

Qualifications

• Completion of a state approved certified nursing assistant course or equivalent training.
• Nursing Assistant Certified (NAC)
• Current Healthcare Provider CPR certification (hands-on demonstration required)
• Current Food Handlers Permit
• High school diploma or GED
• Ability to read and follow written instructions and document care given.
• Understands hospice philosophy and cares for terminally ill patients.
• Prior hospice, long-term care, or similar experience preferred.
• Basic EMR documentation skills
• At least 18 years of age
• Licensed driver with insured vehicle in good working order

Responsibilities

• Perform personal care after assigned tasks and respond promptly to patient requests.
• Provide patient and family with respectful communication and confidentiality.
• Ensure safety and proper use of equipment.
• Assist with baths, back rubs, oral hygiene, and changing bed linens.
• Help with dressing and undressing.
• Assist in feeding; avoid homemade foods prepared by staff.
• Monitor skin condition to prevent tissue breakdown.
• Report pain or changes to RN Case Manager promptly.
• Assist in ambulation and prescribed exercises.
• Provide ROM and transfers as directed.
• Assist with self-administered medications as allowed by state regulations.
• Assist with bedpans, urinals, and light housekeeping tasks.
• Perform ROM and simple procedures with competency as ordered.
• Provide respite for patient/family when on-site.
• Maintain a clean, orderly living area.
• Follow organization documentation and care procedures.
• Participate in quality assessment and performance improvement activities.
• Respect patient and family environment and personal needs.
• As a mandatory reporter, report DOH compliance issues within 14 days.
• Report suspected abandonment, abuse, financial exploitation, or neglect immediately.
