Job summary

Affinity Health Management is seeking a Hospice Aide to work under an RN, performing assigned tasks per the patient’s plan of care to promote comfort and meet personal needs. You will document observations and care provided, and participate in care coordination for quality and safety.

Responsibilities include personal care, feeding support, safety procedures, and assisting with mobility, ROM, and transfers as directed.

Qualifications

• Completion of state approved CNA course.
• Nursing Assistant Certified (NAC) credential.
• Current Healthcare Provider CPR certification (hands-on demonstration).
• Current Food Handlers Permit.
• High school diploma or GED (or equivalent).
• Ability to read and follow written instructions and document care given.
• Understands hospice philosophy and comfort with terminally ill care.
• 1 year prior home health/hospice experience preferred.
• Basic computer skills to document in EMR.
• At least 18 years of age.
• Licensed driver with insured vehicle.

Responsibilities

• Perform personal care tasks and attend to patient requests promptly.
• Provide compassionate communication with patient and family while preserving confidentiality.
• Ensure patient safety and use equipment properly.
• Assist with baths, grooming, oral hygiene, and changing bed linens.
• Assist in dressing, feeding, and mobility as assigned.
• Record and report observations to the RN Case Manager and monitor patient condition.
• Support with self-administered medications as allowed by state regulations (reminders).
• Keep patient living area clean and orderly; assist with light housekeeping tasks.
• Participate in care documentation and quality improvement activities.
• Maintain respectful care environment and comply with department reporting requirements.
