Job summary

The Lakes Home Care, Inc. is seeking a compassionate Home Health Aide to provide daily living and personal care in patients' homes according to an established plan of care.

Responsibilities include bathing, oral hygiene, dressing, assisting with meals, safe transfers, observing patient condition, and reporting changes to the nurse supervisor; and documenting care as directed. The role offers part-time hours with flexible scheduling and a patient-centered team environment.

Qualifications

• HHA Certification required.

• Safe transfer and ambulation techniques.
• Patient's laundry.
• Meeting safety needs of patients and using equipment safely and properly.
• Observation and teaching of home safety practices.
• Recognition of an emergency situation and knowledge of emergency management procedures.
• Follow the plan of care and complete all tasks assigned.
• Communicate clearly with the client, family/caregiver and office.
• Observe and document patient condition and report changes to the assigned nurse supervisor.

• Responsible for assisting patients with and per plan of care, the following:
  

  • Baths, Oral hygiene, Dressing and undressing, Toileting activities, feeding, etc.

  • Safe transfer and ambulation techniques

  • Patient’s laundry

  • Meeting safety needs of patients and using equipment safely and properly (foot stools, side rails, etc.)

  • Observation and teaching of home safety practices

  • Recognition of an emergency situation and knowledge of emergency management procedures

  • Must follow patients’ plan of care and complete all tasks assigned.

  • Communicates in a constant, clear, and effective way with the client, family/caregiver and office

  • Observes and documents patient condition and reports any changes, to the assigned nurse supervisor
