Job summary

The Lakes Home Care, Inc. is seeking a compassionate Home Health Aide to assist patients at home with daily living and personal care according to a care plan.

You will support baths, dressing, feeding, monitoring vitals, safety, and documentation, while following the patient's plan of care and communicating with the nurse supervisor and family. This part-time role offers flexible hours and a focus on quality, dignity, and independence for clients.

Qualifications

• Meets all training requirements in accordance with state and federal laws when applicable
• Ability to read, follow written instructions and document care given
• Possess and maintains current CPR certification in accordance with state regulation
• PPD or chest x-ray if applicable
• Successful completion of Maxim screening examinations
• Participation and completion of Maxim’s competency program
• Must meet all federal, state, and local requirements

Responsibilities

• Assist with baths, oral hygiene, dressing and undressing, toileting activities, feeding and other daily living tasks as per plan of care
• Take and record temperatures, pulse, respiration and blood pressure when ordered (with competency)
• Assist with safe transfers and ambulation
• Support patient exercise according to plan of care
• Perform ROM and other simple procedures as ordered
• Assist with patient laundry
• Ensure safety and proper use of equipment
• Observe and teach home safety practices
• Recognize emergencies and follow emergency procedures
• Follow patient plans of care and complete assigned tasks
• Communicate clearly with client, family/caregiver and office
• Observe and document patient condition and report changes to nurse supervisor
