Job summary

The Lakes Home Care, Inc. is seeking a compassionate Home Health Aide to assist patients with daily living and personal care in their homes according to a care plan.

Responsibilities include bathing, grooming, mobility assistance, measuring vitals as ordered, safe transfers, ROM as directed, and documenting changes for the supervising nurse. Flexible, part-time schedule with competitive pay and support from a dedicated team.

Qualifications

• Meets training requirements per state and federal laws.
• Ability to read, follow written instructions and document care given.
• Current CPR certification in accordance with state regulation.
• PPD or chest x-ray if applicable.
• Successful completion of Maxim screening examinations.
• Participation in Maxim’s competency program.

Responsibilities

• Assist patients with baths, oral hygiene, dressing, toileting, feeding, and related daily activities.
• Take and record vitals and report changes to supervising nurse.
• Assist with safe transfers and ambulation; provide ROM and simple procedures as ordered.
• Handle patient laundry and maintain safety with equipment.
• Observe, document, and communicate patient condition to the nurse supervisor.
