Job summary

The Lakes Home Care, Inc. is seeking a compassionate Home Health Aide to assist patients in their homes according to an established care plan. You will support daily living activities, monitor health, and communicate changes to the supervising nurse.

Responsibilities include baths, hygiene, mobility, safety, and documenting patient conditions. The position offers flexible, part-time hours and competitive pay, with the chance to join a dedicated team delivering high-quality home care.

Qualifications

• Meets training requirements per state and federal laws where applicable.
• Ability to read, follow written instructions and document care given.
• possesses and maintains CPR certification according to state regulation.
• PPD or chest X-ray if applicable.
• Successful completion of Maxim screening examinations and Maxim competency program.

Responsibilities

• Assist patients with baths, oral hygiene, dressing, toileting, feeding according to care plan.
• Take and record vital signs as ordered (temperature, pulse, respiration, BP).
• Support safe transfers and ambulation; assist with exercise per plan of care.
• Perform ROM and simple procedures as instructed; assist with patient laundry.
• Observe safety needs, use equipment properly, and teach home safety practices.
• Document patient condition and report changes to the supervising nurse.
