Job summary

The Lakes Home Care, Inc. is seeking a compassionate Home Health Aide to provide daily living and personal care in patients' homes in accordance with an established care plan. This part-time role offers flexible scheduling and the opportunity to support patients in their own homes.

Responsibilities include assisting with baths, hygiene, dressing, transfers, and documenting care. Applicants should be CPR certified and able to read and follow care plans, with competency in safety practices.

Qualifications

• Meets training requirements in accordance with state/federal laws when applicable.
• Ability to read, follow written instructions and document care given.

Responsibilities

• Assist patients with daily living tasks per care plan (baths, hygiene, dressing).
• Monitor and document patient vitals when ordered.
• Ensure safe transfers and ambulation.
• Provide range of motion exercises as directed.
• Handle laundry and light housekeeping per plan.
• Observe patient condition and report changes to nurse.
• Communicate clearly with patient, family, and office.
• Meet safety needs and use equipment properly.
