Job summary

Qualifications

• CPR certification current and in accordance with state regulations.
• Able to read, follow written instructions and document care.
• Complete Maxim competency program and screenings.

Responsibilities

• Assist patients with the plan of care including baths, hygiene, dressing, toileting, feeding.
• Record vital signs and document observations per protocol.
• Assist with safe transfers, ambulation, and exercises as ordered.
• Provide home safety guidance and patient education.
• Communicate clearly with client, family, and nurse supervisor.
• Observe patient condition and report changes promptly.
