Job summary

The Lakes Home Care, Inc. is seeking a compassionate Home Health Aide to provide daily living and personal care to patients in their homes, following an established care plan.

Responsibilities include assisting with baths, hygiene, dressing, toileting, feeding, and taking vital signs as ordered, while safely transferring and guiding exercises and maintaining patient safety and accurate documentation for the supervising nurse.

Part-time schedule with flexible hours and a supportive team.

Qualifications

• Meets all training requirements in accordance with state and federal laws when applicable.
• Ability to read, follow written instructions and document care given.
• Possess and maintains current CPR certification in accordance with state regulation.
• Completion of Maxim screening examinations.
• Participation and completion of Maxim’s competency program.

Responsibilities

• Assist patients with daily living activities per plan of care, including baths, hygiene, dressing, toileting, and feeding.
• Take and record vital signs as ordered (oral, rectal, axillary, when instructed).
• Perform safe transfer and ambulation techniques; assist with ROM and prescribed exercises.
• Complete patient laundry and maintain home safety practices.
• Observe and document patient condition; report changes to the assigned nurse supervisor.
