Job summary

The Lakes Home Care, Inc. is seeking a compassionate Home Health Aide to provide daily living and personal care in patients' homes in accordance with an established care plan.

Responsibilities include assisting with baths, dressing, feeding, and safety checks; taking vital signs when ordered; transfers and ROM; patient laundry; and reporting changes to the nurse supervisor. Strong communication with patients and caregivers is essential.

Qualifications

• Meets training requirements per state and federal laws when applicable.
• Ability to read, follow written instructions and document care given.
• Possess and maintain current CPR certification as required by state regulation.
• PPD or chest x-ray if applicable.
• Successful completion of Maxim screening examinations.
• Participation and completion of Maxim’s competency program.

Responsibilities

• Assist patients with plan of care: baths, oral hygiene, dressing, toileting, feeding, etc.
• Take and record temperatures, pulse, respiration and blood pressure when ordered.
• Safe transfer and ambulation; assist with exercises per care plan.
• Perform ROM and simple procedures as ordered.
• Handle patient laundry and maintain safety with equipment.
• Observe and document patient condition; report changes to nurse supervisor.
• Communicate clearly with client, family/caregiver and office.
• Follow plan of care and complete all tasks assigned.
• Recognize emergencies and apply appropriate management procedures.
