Job summary

Qualifications

• Must pass fingerprint clearance and background checks.
• HS/GED; VOALA Case Management training within 6 months.
• Minimum 1 year case management or related social services experience, or a Bachelor's in a related field.
• Preferred: Bachelor's in Social Work or related field; HMIS navigation experience.

Responsibilities

• Conduct comprehensive screenings and assessments to collect functional, environmental, psycho-social, financial, employment, housing, educational, and health information.
• Assess barriers facing clients and develop a case plan.
• Coordinate planning with clients to meet short- and long-term needs.
• Provide service linkages and support systems to ensure identified needs are met.
• Advocate for clients before judicial, community, social service, and administrative bodies as needed.
