Job summary

Volunteers of America Los Angeles is seeking a Case Manager (Specialist) to deliver intensive, on-site case management services to eligible clients with weekly contact. You will coordinate services, develop action plans, and act as an advocate across multiple systems to meet short- and long-term goals.

The role emphasizes a thorough intake, ongoing support, and coordination with the VOALA team. Responsibilities include screenings, service linkages, and group education while ensuring compliance

Qualifications

Must have knowledge of case management processes and resources.
Able to coordinate services and advocate for clients across agencies.
Strong communication and documentation skills.

Responsibilities

Conduct comprehensive screenings and assessments to build a client-centered plan.
Provide service linkages and support systems to meet identified needs.
Coordinate individualized planning and case conferences with the service team.
Monitor progress and adjust care plans to meet goals.
Advocate for clients before judicial, community, and social service bodies.
Document group education sessions clearly and concisely.
