Job summary

VOALA is seeking a Case Manager Generalist to serve low acuity clients at on-site locations in the Los Angeles area. You will coordinate services, conduct screenings, and develop individualized case plans to meet immediate and long-term needs.

Responsibilities include advocating for clients, coordinating care with service teams, and connecting participants to housing, employment, and health resources. HS/GED required; 1 year experience preferred. E-Verify compliant.

Qualifications

• HS/GED; VOALA Case Management training within 6 months of hire.
• Minimum 1 year general experience in case management or related field.
• Or a Bachelor’s Degree in Social Services related field in lieu of experience.

Responsibilities

• Conduct screenings and assessments to collect social, health, housing, and financial information.
• Develop client case plans and coordinate services to meet goals.
• Advocate for clients with external agencies and service providers.
• Coordinate care conferences and monitor progress on the case plan.
• Provide linkages to housing, employment, and health resources.
