Job summary

Volunteers of America Los Angeles is seeking a Case Manager Generalist to deliver on-site case management services to low-acuity clients and coordinate needed resources within VOALA's service framework.

You will conduct screenings, develop case plans, advocate for clients with various agencies, and connect them to housing, employment, and health resources. Requires fingerprint clearance and HS/GED; VOALA training within 6 months; 1 year of experience or related degree.

Qualifications

• Must pass fingerprint clearance and background check.
• HS/GED required; VOALA Case Management training within 6 months.
• Minimum 1 year of case management or related social services experience.

Responsibilities

• Conduct screenings and assessments to collect functional, environmental, psycho-social, financial, employment, housing, educational, and health information.
• Develop and coordinate case plans with clients to meet short- and long-term needs.
• Coordinate case conferences with residents and service teams; monitor progress of case plans.
• Advocate for clients before agencies and community bodies; facilitate support groups as needed.
• Provide coaching, social support, and link clients to appropriate services.
