Job summary

Volunteers of America Los Angeles is seeking a Case Manager Generalist to provide intake, assessment and coordinated services to clients with low acuity. You will serve as the primary contact to connect individuals with housing, employment, healthcare and social supports.

Responsibilities include conducting comprehensive screenings, developing individualized case plans, coordinating services with the care team, and advocating for clients with community and service providers.

Qualifications

• Must pass fingerprint clearance and background checks.
• HS/GED; VOALA Case Management training within 6 months of hire.
• Minimum 1 year general experience in case management or related field; or a Bachelor’s Degree in Social Services.
• HMIS navigation experience is preferred.

Responsibilities

• Conducts comprehensive screenings and assessments and collects functional, environmental, psycho-social, financial, employment, housing, educational, and health information.
• Develops case plans and coordinates services with community resources to meet client needs.
• Advocates for clients and liaises with judicial, social service and community bodies as needed.
