Job summary

Volunteers of America Los Angeles (VOALA) is hiring a Case Manager Generalist to provide on-site case management services to clients with low acuity in our Los Angeles programs. You will assess needs, develop individualized plans, coordinate services, advocate for clients, and connect them with housing, health, and social supports.

The role emphasizes direct client contact, collaboration with a multidisciplinary team, mandated reporting, and delivering compassionate support to help clients

Qualifications

• Minimum 1-year general experience in case management or similar social services role.
• HS/GED. Completion of VOALA Case Management training within 6 months of hire.
• Ability to pass a fingerprint clearance and background check.
• Experience coordinating services and advocating for clients.

Responsibilities

• Conduct screenings and assessments to collect required information and develop a plan.
• Coordinate individualized planning with clients to meet short- and long-term needs.
• Provide service linkages and support systems to ensure identified needs are met.
• Coordinate case conferences, monitor progress, and assist clients to meet goals.
• Advocate for clients before agencies and community settings; facilitate groups.
• Engage in outreach and ensure on-site service delivery as required.
