Gentiva Hospice is seeking an After Hours / On-Call Hospice RN to respond to patient needs during evenings, nights, weekends, and holidays. You will deliver direct patient care, coordinate with the daytime team, and guide symptom management in patients' homes and facilities.

Qualifications

• Current RN license in the state of employment.
• 1+ years of nursing experience.
• Hospice heart: compassionate, calm, and dependable in urgent care situations.
• Excellent clinical judgment for emotionally intense settings.
• Comfort delivering care in patients' homes or other settings.
• Valid driver’s license, car insurance, and reliable transportation.
• Current CPR certification.

Responsibilities

• Provide direct patient care and support after normal business hours.
• Serve as the primary liaison between patients, families, physicians, and the hospice team during your shift.
• Respond promptly to urgent calls, patient visits, and symptom management needs.
• Evaluate patient and family needs to guide clinical decisions and implement the plan of care.
• Document clinical visits and communications accurately.
• Collaborate with an interdisciplinary group to deliver comprehensive care.
