Job summary

Gentiva Hospice in Warwick, Rhode Island seeks an After Hours Registered Nurse (RN) to provide compassionate care in the home setting during evenings, nights, weekends and holidays. You will respond to urgent calls, assess patients, and coordinate with the hospice team to implement the plan of care.

Qualifications

• Current RN license in RI or eligibility to obtain.
• 1+ years of nursing experience.
• Hospice, home health, oncology, or palliative care experience preferred.
• CPR certification.
• Valid driver’s license and reliable transportation.
• Strong documentation and EMR skills.

Responsibilities

• Respond to patient needs after hours, evenings, weekends and holidays.
• Provide direct patient care and support during shifts.
• Act as primary liaison between patients, families, physicians, and the hospice team.
• Document clinical visits and communications accurately.
• Participate in quality assurance and performance improvement programs.
• Maintain continuity of care and coordinate with daytime team.
