Job summary

Gentiva Hospice is seeking an After Hours Hospice RN to provide compassionate in-home care during evenings, nights, weekends, and holidays. You will be the primary liaison for patients, families, and the hospice team, delivering urgent symptom management and tailored care plans.

Ideal candidates bring an RN license in the state of employment, 1+ years of nursing experience, CPR certification, and a calm, dependable demeanor under pressure.

Qualifications

• Current RN license in the state of employment (or eligibility).
• 1+ years of nursing experience.
• Compassionate, calm, and dependable in urgent care situations.
• Excellent clinical judgment and ability to provide support in emotionally intense settings.
• Comfort delivering care in patients' homes, facilities, or wherever they reside.
• Valid driver’s license, automobile insurance, and reliable transportation.
• Current CPR certification.

Responsibilities

• Provide direct patient care after normal hours.
• Serve as the primary liaison between patients, families, physicians, and the hospice team during your shift.
• Respond promptly to urgent calls, patient visits, and symptom management needs.
• Evaluate patient and family needs to guide clinical decisions and implement the plan of care.
• Collaborate with LPNs/LVNs, CNAs, and other support staff.
• Accurately document clinical visits and communications.
• Participate in quality assessment and performance improvement programs.
• Work independently while coordinating with daytime care team.
