Job summary

Gentiva Hospice is seeking an experienced RN to provide after-hours, on-call hospice nursing in Harrisonburg, Virginia. The role focuses on compassionate, patient-centered care during evenings, nights, weekends and holidays, with strong collaboration with the hospice team.

You will assess patients, manage symptoms, document care, and support families while coordinating with physicians and the daytime staff to ensure seamless continuity of care.

Qualifications

• Current RN license in the state of employment
• 1+ year nursing experience, preferably in hospice or home health
• Ability to work after hours, weekends and holidays
• Strong clinical judgment and compassionate patient care
• Experience with EMR and documentation practices

Responsibilities

• Provide direct patient care after normal business hours in home or facility settings
• Serve as primary liaison between patients, families, physicians and hospice team
• Respond to urgent calls, perform symptom management and implement care plans
• Document visits accurately and maintain patient records
• Collaborate with interdisciplinary team and participate in QA/PI programs
• Maintain continuity of care with daytime team and attend staff meetings
