Gentiva is seeking a dedicated Afterhours RN Care Manager to provide high-quality hospice care outside standard business hours. This role involves assessing patient needs during after-hours shifts and ensuring continuity of care.

Qualifications

• Minimum one year of RN experience in acute care, oncology, long-term care, hospice, or home health.
• Must meet any state-specific requirements.

Responsibilities

• Assess and coordinate patient and family needs during after-hours shifts.
• Provide direct in-person patient care during on-call, weekend, and after-hours shifts.
• Educate patients and families on disease progression and hospice philosophy.

Skills

Strong knowledge of hospice principles

Effective communication

Education

Current RN license in the state of employment
