Job summary

Gentiva seeks an Afterhours RN Care Manager to deliver high-quality hospice care outside standard hours, including nights and weekends. You will provide direct patient care, conduct assessments, and manage admissions and live discharges while maintaining clinical oversight.

You will collaborate with physicians, families, and the IDG to ensure continuity of care, timely documentation, and adherence to hospice guidelines. An active RN license and 1+ year of RN experience are required.

Qualifications

• Current RN license in the state of employment.
• Graduate of an accredited nursing program
• Minimum one year of RN experience in acute care, oncology, long-term care, hospice, or home health
• Must meet any state-specific requirements

Responsibilities

• Provide direct in-person patient care during after-hours shifts, including assessments and visits.
• Coordinate with IDG and physicians to ensure continuity of care.
• Maintain accurate clinical documentation and care plans.
• Educate patients and families on disease progression and hospice care.
• Ensure infection control and compliance with regulatory standards.
