Job summary

Gentiva Hospice seeks an After Hours / On-Call Hospice RN to respond to patient needs during evenings, nights, weekends, and holidays in home and facility settings. You will serve as the primary liaison among patients, families, physicians, and the hospice team while ensuring compassionate, high-quality care.

You must hold a current RN license in the state of employment, have 1+ year of nursing experience, and possess CPR certification. Valid driver's license and EMR familiarity are required.

Qualifications

• Current RN license in the state of employment or eligibility to obtain
• 1+ years of nursing experience
• Compassionate, calm, and dependable in urgent care situations
• Excellent clinical judgment and ability to provide support in emotionally intense settings
• Comfortable providing care in patients' homes, facilities, or wherever they reside
• Valid driver's license, automobile insurance, and reliable transportation
• Current CPR certification

Responsibilities

• Report directly to the Administrator, Executive Director, or Patient Care Manager Senior and provide direct patient care and support after normal business hours
• Serve as the primary liaison between patients, families, physicians, and the hospice team during your shift
• Respond promptly and compassionately to urgent calls, patient visits, and symptom management needs
• Evaluate patient and family needs to guide clinical decisions and implement the plan of care
• Support and collaborate with an interdisciplinary group including LPNs/LVNs, CNAs, and other support staff
• Accurately document clinical visits and communications
• Participate in quality assessment and performance improvement programs
• Work independently while ensuring continuity of care and coordination with the daytime care team
