Job summary

Gentiva is seeking an After Hours Hospice RN to respond to patient needs during evenings, nights, weekends, and holidays, delivering compassionate care when it’s most needed. You will work Monday 5pm - Thursday 8am, collaborating with the daytime team and an interdisciplinary group to ensure continuity of care.

The role requires an RN license in the state of employment, 1+ year of nursing experience, and the ability to work in patients’ homes or facilities with strong clinical judgment and

Qualifications

• Current RN license in the state of requested employment (or eligibility to obtain).
• 1+ years of nursing experience.
• Hospice care mindset: compassionate, calm under pressure.
• Strong clinical judgment and ability to support in emotionally intense settings.

Responsibilities

• Provide direct patient care and support after normal business hours.
• Serve as primary liaison between patients, families, physicians, and the hospice team during shifts.
• Respond promptly to urgent calls, patient visits, and symptom management needs.
• Evaluate patient and family needs to guide clinical decisions and implement the plan of care.
• Document visits and communications accurately and participate in quality improvement programs.
