Job summary

Gentiva is seeking a Registered Nurse (RN) for Hospice care in Newport News, Virginia. The After Hours RN will respond to patient needs during evenings, nights, weekends, and holidays, delivering compassionate care and ensuring continuity of the plan of care.

Responsibilities include direct patient care after hours, acting as liaison among patients, families, physicians, and the hospice team, and documenting all visits and communications within the EMR.

Qualifications

• Current RN license in the state of employment (or eligibility).
• 1+ years of nursing experience.
• CPR certification required.
• Hospice/home health experience preferred.
• Strong documentation and EMR skills.

Responsibilities

• Provide direct patient care after normal business hours.
• Serve as liaison between patients, families, physicians, and hospice team.
• Respond to urgent calls, visits, and symptom management needs.
• Document clinical visits and communications accurately.
• Collaborate with interdisciplinary team and participate in QA/PI programs.
• Maintain continuity of care and coordination with daytime team.
