• Provide Case Management services to individuals with intellectual or developmental disabilities.
• Establish and monitor Person-Centered Plans (PCPs).
• Coordinate resources and services to meet PCP outcomes.
• Advocate for individuals and educate families about services.
• Ensure HIPAA compliance and maintain confidential records.

Education

Bachelor's degree (human services) OR Bachelor's degree in non-human services with two years of human services experience
