Molina Healthcare, Inc. is seeking a Community Health Worker to support community-based member advocacy activities and serve as a local member resource.

The role involves field visits to members in homes, shelters, and provider offices, helping them navigate health plan benefits and access social services. The position requires 1 year of health care experience with underserved populations, valid driver's license, and strong communication skills.

Qualifications

• At least 1 year of health care experience with underserved populations.
• CHW certification may be required in certain states.
• Valid driver's license, reliable transportation, and adequate auto insurance for travel.
• Ability to multi-task applications while speaking with members.
• Excellent customer service, organizational and time-management skills.
• Excellent verbal and written communication skills.
• Proficiency with Microsoft Office and relevant software.

Responsibilities

• Engages with members as an advocate and resource to support management of health care needs.
• Collaborates with the health care services team by providing non-clinical paraprofessional duties in the field.
• Helps members navigate and maximize health plan benefits and coordinate care.
• Assists members in accessing social services and community resources.
• Provides outreach to reconnect disconnected members with special needs.
• Performs outreach and research to locate and contact hard-to-reach members.
• Supports Medicaid eligibility guidance and related financial resources.
• Travels locally 50-80% as required.
