Job summary

Qualifications

• At least 1 year of health care experience or equivalent.
• CHW certification may be required for certain states.
• Valid driver's license and reliable transportation for travel.

Responsibilities

• Engages with members as an advocate and resource to support health care management.
• Supports health care services team with non-clinical paraprofessional duties in the field.
• Helps members navigate health plan benefits and schedule appointments.
• Assists members in accessing social services and community resources.
• Provides outreach to locate and support disconnected members with special needs.
• Conducts research to locate members using databases and community resources.
• Participates in outreach activities, telephonic and in-person.
• Guides members to maintain Medicaid eligibility and financial resources.
• Travel of 50-80% may be required.
