Job summary

Molina Healthcare in the United States seeks a Community Health Worker to support community-based member advocacy, engage vulnerable members, and connect them with health plan benefits and social services.

You will travel locally 50-80% to meet members in homes, shelters, provider offices, and community sites, assisting with appointments, transportation, and care coordination as part of the healthcare team.

Qualifications

• At least 1 year of health care experience with underserved populations.
• CHW certification may be required for certain states.
• Valid driver's license, reliable transportation and adequate auto insurance.
• Excellent customer service, organizational and time-management skills.
• Ability to learn additional software systems and applications.
• Excellent verbal and written communication; proficient in Microsoft Office.

Responsibilities

• Advocate for members and help manage health care needs.
• Provide non-clinical paraprofessional support in the field.
• Assist with scheduling, transportation, prescriptions, and follow-ups.
• Help members access social services and community resources.
• Conduct outreach to reconnect disconnected members with special needs.
• Perform research to locate hard-to-contact members and coordinate outreach.
• Participate in outreach activities, telephonic or in-person.
• Guide members to maintain Medicaid eligibility and other resources.
