Molina Healthcare is seeking a Community Health Worker to support local member advocacy and connect vulnerable members with available resources. The role emphasizes non-clinical field work, home visits when needed, and helping members navigate health plan benefits and social services.

Qualifications

• At least 1 year health care experience with underserved populations.
• CHW certification may be required for certain states.
• Valid driver's license, reliable transportation, and adequate auto insurance for travel requirements.
• Ability to multi-task applications while speaking with members.
• Excellent customer service skills.
• Organizational and time-management skills.
• Ability and willingness to learn other programs and software systems.

Responsabilités

• Engages with members as an advocate and resource to support health care needs.
• Collaborates with health care services team with non-clinical duties in the field (home visits, offices, shelters).
• Helps members navigate and maximize health plan benefits and coordinate appointments, transportation, and prescriptions.
• Assists members in accessing social services and community resources (housing, food, employment).
• Provides outreach to locate and support disconnected members with special needs.
• Performs research to locate members not reachable via standard channels.
• Participates in ongoing or project-based member outreach activities (telephonic or in person).
• Guides members to maintain Medicaid eligibility and other financial resources.
• Travel 50-80% locally as required by state/contract.
