Molina Healthcare seeks a Community Health Worker to support community-based member advocacy and engage vulnerable members in managing health care needs. You will work as a local resource, guiding members through their health plan benefits and connecting them with essential services.

The role involves non-clinical paraprofessional duties in the field, home visits, and collaboration with care teams. Strong communication and organizational skills are essential.

Competenze

• At least 1 year of health care experience serving underserved populations.
• CHW certification may be required for certain states.
• Valid and unrestricted driver\'s license with reliable transportation for travel.
• Ability to multi-task applications while speaking with members.
• Excellent customer service skills and communication abilities.
• Proficiency in Microsoft Office and related software.

Mansioni

• Advocate for members and support health care needs management.
• Collaborate with health care services team on non-clinical field duties.
• Assist members in navigating benefits and scheduling appointments.
• Help members access social services such as housing, food, and employment.
• Reach out to disconnected members and provide support.
