Job summary

Molina Healthcare is hiring a Community Health Worker to support community-based member advocacy activities. You will engage and assist vulnerable members in managing healthcare needs, leveraging local resources and community knowledge to improve quality and cost-effective care.

The role requires local travel (50-80%), collaboration with care teams, and a focus on navigating health plans and social services. Experience with underserved populations and a CHW certification is valued.

Qualifications

• 1+ year health care experience working with underserved populations.
• CHW certification may be required for certain states.
• Valid and unrestricted driver’s license, reliable transportation and adequate auto insurance for travel.
• Ability to multi-task applications while speaking with members.
• Excellent customer service skills.
• Organizational and time-management skills.
• Willingness to learn other programs and software systems.

Responsibilities

• Engages with members as an advocate and resource to support health care needs.
• Collaborates with health care services team providing paraprofessional duties in the field.
• Helps navigate and maximize health plan benefits: scheduling, transportation, prescriptions, follow-ups.
• Assists members in accessing social services (housing, food, employment).
• Provides outreach to locate and support disconnected members with special needs.
• Researches available data to locate uncontacted members via databases or community resources.
• Participates in ongoing or project-based outreach activities (telephonic and/or in-person).
• Guides members to maintain Medicaid eligibility and other financial resources.
