Molina Healthcare is seeking a Community Health Worker to support community-based member advocacy activities. The role acts as a local resource, helping vulnerable members manage health care needs by navigating benefits, arranging transportation, scheduling appointments, and connecting to social services.

The CHW collaborates with the health care services team to deliver non-clinical paraprofessional support at member homes, shelters, and provider offices.

Qualifications

• At least 1 year of health care experience with underserved populations.
• CHW certification may be required for certain states depending on contractual requirements.
• Valid and unrestricted driver's license with reliable transportation for job travel.
• Ability to multi-task applications while speaking with members.
• Excellent customer service, verbal and written communication skills.

Responsabilités

• Engages with members as an advocate and resource to support management of health care needs.
• Collaborates with the health care services team by providing non-clinical paraprofessional duties in the field (homes, shelters, offices).
• Assists members in navigating and maximizing health plan benefits, scheduling appointments, and arranging transportation.
• Helps members access social services such as housing, food, employment and other resources.
• Supports outreach to locate and assist disconnected members with special needs.
• Conducts research to locate members not reached by Molina's contact efforts.
• Participates in ongoing or project-based activities requiring extensive member outreach.
• Guides members to maintain Medicaid eligibility and access financial resources.
