Job summary
A health care organization is seeking a Community Health Worker to engage members and provide education on health resources. This fully remote position requires a High School Diploma/GED and 1+ years in clinical or case management roles. Responsibilities include assessing member needs, coordinating care, and addressing social determinants of health. The role offers a pay range of $20.38 - $36.44 per hour and a comprehensive benefits package, including paid time off and medical plans.

Qualifications
- 1+ years of clinical or case management experience.
- Intermediate level of proficiency with computers and Microsoft Office (Word, Excel, and Outlook).
- Ability to work a shift between the hours of 8:00 am - 5:00 pm CST.
Responsibilities
- Engages members primarily over the phone to discuss their health.
- Creates a positive experience and relationship with the members.
- Supports the members to improve their well-being by staying out of the hospital.
