Upward Health is seeking a Community Outreach Specialist to establish its presence in Warwick and surrounding communities. You will educate potential patients about our in-home, whole-person care offerings and inform them about enrollment opportunities.

In this role you will manage a personal caseload, primarily using phone outreach with occasional in-person visits and other community-based strategies. You will work closely with the Outreach Manager and care teams to improve patient engagement.
