Job summary

Upward Health is seeking a Community Outreach Specialist to establish the company’s presence in the community and educate potential patients about our in-home, whole-person care services. This role involves phone outreach, in-person visits, and various community-based strategies to engage individuals and enroll them into programs.

The COS will manage a personal caseload, report to the Outreach Manager, and work with interdisciplinary teams to ensure successful outreach and patient onboarding.

Qualifications

• Excellent verbal and written communication skills.
• Ability to build trust and rapport quickly with patients.
• Strong organizational skills to manage a personal caseload.

Responsibilities

• Educate patients about Upward Health’s services and programs.
• Conduct outreach via phone, in-person visits, and mailings.
• Facilitate enrollment into programs and coordinate onboarding.
• Maintain accurate documentation in patient records.
• Collaborate with Outreach Manager and team members.
