Job summary

Upward Health is seeking a Community Outreach Specialist to establish the company’s presence in the community and engage potential patients. You will educate individuals about services, enroll them into programs, and manage a personal caseload using phone outreach, in-person visits, and community strategies.

You will report to the Outreach Manager and collaborate with team members to ensure successful outreach and patient engagement across diverse communities.
