Job summary

Upward Health is seeking a Community Outreach Specialist to expand our presence in the community and educate potential patients about our in-home, whole-person care model. The COS will manage a personal caseload, primarily via phone outreach, and may conduct in-person visits and other community-based activities to enroll patients into our programs.

The COS reports to the Outreach Manager and collaborates with team members to ensure successful outreach and engagement across diverse communities.

• Strong organizational and multitasking abilities to manage a personal caseload.
• Self-motivated with ability to work independently and meet outreach goals.

• Flexible to use phone, in-person visits, and mailings for outreach.
• Fluent in English; Spanish proficiency is a plus.
