Job summary

Upward Health, Inc. is hiring a Community Outreach Specialist to establish the company’s presence in the community and engage potential patients. The COS will educate individuals about Upward Health’s offerings, manage a personal caseload, and drive enrollment via phone outreach, in-person visits, and community-based strategies.

The role reports to the Outreach Manager and collaborates with the team to maximize patient outreach success and program enrollment.

Responsibilities

• Manage a personal caseload and conduct phone outreach.
• Conduct in-person visits and other community-based outreach activities as needed.
• Educate potential patients about Upward Health services and programs.
• Facilitate enrollment into Upward Health programs and ensure smooth onboarding.
