Job summary

Upward Health in Providence seeks a Community Outreach Specialist to establish our presence in the community, educate potential patients about our services, and guide enrollment into programs. The COS will manage a personal caseload, primarily using phone outreach, with occasional in-person visits and other outreach strategies as needed.

You will collaborate with a multidisciplinary team to ensure patients understand available services and can take the first steps toward improving their health,

Responsibilities

• Educate patients about Upward Health’s services and enroll them into programs.
• Manage a personal caseload and conduct outreach primarily by phone, with in-person visits as needed.
• Document interactions accurately in company systems and track follow-ups.
• Collaborate with the Outreach Manager and team to optimize engagement strategies.
