Job summary

Upward Health, Inc. is seeking a Community Outreach Specialist who will be the first point of contact for potential patients. You’ll educate them about services, engage in meaningful conversations, and help enroll them in our programs.

You’ll manage a personal caseload primarily by phone while occasionally visiting communities and patients in-person. The role requires strong communication, rapport-building, and organizational skills.

Qualifications

• Experience in community outreach or patient enrollment preferred.
• Ability to communicate Upward Health’s services clearly to diverse audiences.

Responsibilities

• Educate patients about Upward Health’s offerings and programs.
• Engage individuals in meaningful conversations and guide enrollment.
• Manage a personal caseload using phone outreach, with occasional in-person visits.
• Coordinate with Outreach Manager and team to ensure outreach goals are met.
