Job summary

Oak Street Health is seeking a Clinical Informatics Specialist to support primary care providers with real-time documentation, join them in patient visits, and prepare after-visit summaries. This role emphasizes accurate documentation within our value-based care model, including ICD-10 and CPT coding.

Candidates should have medical terminology knowledge, strong listening and communication skills, and the ability to learn new workflows.

Qualifications

• Knowledge of medical terminology and common medications.
• Shadowing or volunteering in clinical settings is preferred.
• Scribe or transcription experience is a plus.

Responsibilities

• Document patient encounters and assist with CPT/ICD-10 coding.
• Join providers in exams and prepare after visit summaries.
• Support clinical documentation improvement and records requests.
• Assist with daily huddles and care team collaboration.
• Provide in-person administrative support and referrals as needed.
