Job summary

Oak Street Health is seeking a Clinical Informatics Specialist (CIS/Medical Scribe) to support primary care providers with real-time documentation and coding. You will join a care team observing visits, documenting encounters, and preparing after-visit summaries across clinic settings.

Ideal candidates have clinical exposure, strong listening and communication skills, and the ability to learn new workflows while supporting chronic condition documentation and population health data streams.

Qualifications

• Advanced listening and communication abilities to document patient encounters accurately.
• Strong computer literacy and ability to adapt to new workflows in clinical settings.
• Fluency in Spanish, Polish, Russian, or other languages spoken by communities served (where indicated).

Responsibilities

• Document patient encounters in structured notes with history, assessment, plan and exam.
• Assist with CPT and ICD-10 coding and After Visit Summaries.
• Support clinical documentation improvement and review medical records.
• Provide administrative support including task intake, referrals, and care team coordination.
