Job summary

Oak Street Health seeks a Medical Scribe to support primary care providers by documenting patient encounters in real time, using structured notes that cover history, assessment, plan and exam.

You will assign CPT and ICD-10 codes, prepare After Visit Summaries, assist with documentation improvement, and provide administrative support to the care team in a clinic setting that requires HIPAA compliance.

Qualifications

• Knowledge of medical terminology and common medications, either from a pre-medical degree or prior clinical experience [required]
• Prior clinical experience, including shadowing and/or volunteering [strongly preferred]
• Prior scribe or transcription experience [preferred but not required]

Responsibilities

• Document patient encounters in a structured note, including history of present illness, assessment, plan and physical exam
• Assign CPT and ICD-10 codes
• Prepare After Visit Summaries
• Consult with provider to ensure accurate and specific documentation
• Administrative support for the provider and care team
