Job summary

Enhabit Home Health & Hospice seeks a qualified nursing or physical therapy professional to review patient information and ensure documentation meets regulatory standards. The role includes training LPs and collaborating with branch leadership to uphold care quality.

This position emphasizes direct patient care coordination, ongoing staff education, and maintaining strong relationships with patients, families, and referral sources.

Qualifications

• Graduate of an approved school of professional nursing or physical therapy.
• Current license in the state of residence; CPR certification.
• Minimum of one year of experience; Medicare home health or hospice preferred.
• Valid driver's license and auto liability insurance.
• Proficient with Microsoft Word and Excel.

Responsibilities

• Review and approve patient information during start of care, recertification, or evaluation visits.
• Provide ongoing education to LPs and offer one-on-one training when needed.
• Promote quality patient care and maintain relationships with referral sources, patients, caregivers and staff.
